GETTING FROM PRE-ACA TO POST-ACA
This Cartesian Solutions, Inc.™ Service, which is typically contracted as part of one or more of the other client Services, helps government agencies work together in setting priorities and timelines for identified value-added changes in public programs’ health support processes and enrollee assist and support services as they transition from volume-based to value-based enrollee health outcome enhancements.
Up to this point, Cartesian Solutions, Inc.™ services have focused on assistance with: 1) creating a health delivery environment in which public program enrollees can access care most likely to stabilize health thus reducing health service use, 2) introducing value-added medical and behavioral health support for high need and high cost enrollees with comorbid conditions, and 3) connecting service #1 and #2 by facilitating Complex Care Programs for enrollees in the public program service area with the greatest health need and service use. Unfortunately, services #1 through #3 are not turnkey. Changes in health care contracting and services delivery do not happen overnight. Rather, there is prioritization and sequencing of internal public program health-related initiatives, often with timelines measured in years, that are required for government agencies to achieve their ultimate goal of best practices support for total enrollee health.
