DEPLOYING INTEGRATED CASE MANAGEMENT (ICM) PROGRAMS
Three components are needed to effectively deploy non-physician assist and support services, otherwise known by terms, such as care coordination or case management:
- Case manager backgrounds, experience, and program-specific training that allows consistent delivery of assistance and support, which reverses barriers to improvement,
- A program environment in which work processes and reward systems consistently support case managers in achieving health and cost outcomes with their clients, and
- Medical administrator/physician understanding of how to set up ICM programs, including collaborative working relationships between ICM managers and clinicians to maximize measured health outcomes for patients.
Cartesian Solutions, Inc.™ uses the text, The Integrated Case Management Manual: Assisting Complex Patients Regain Physical and Mental Health (Springer Publ, 2010) and a comprehensive Adult and Pediatric Integrated Case Management (ICM) Training Program to prepare Cartesian Solutions, Inc.™ client case managers to deliver value-based assistance and support to their constituencies. This Service couples Cartesian Solutions, Inc.™ ICM Organizational Assessments with ICM training to assure that trained ICM managers are supported and rewarded for bringing maximum benefit to those with whom they work. Finally, the text, Physician’s Guide: Understanding and Working with Integrated Case Managers (Springer, 2016) is used to educate 1) medical administrators on how to maximize value to patients with health complexity in setting up ICM and other case management programs and 2) practitioners in how to reap the greatest benefit from ICM manager assist and support services.
